The parish community of Our Lady of Perpetual Help Parish Cubao will always remember Sr. Alfonsa dela Santissima Trinidad of the Congregation of San Agustin as its original founder and benefactor. In 1946, she bought, with her family inheritance, a piece of land in Murphy Cubao. It was her intention to build an orphanage for street children. She did an orphanage, but it was eventually transformed into a small chapel where catechism classes were held for children, and where a regular sunday mass was celebrated for the community.
This became one of the chapels of the Immaculate Conception Parish in Lantana, under the care of SVD fathers. it was named Our Lady of Perpetual Help Chapel, where the novena to Our Lady of Perpetual Help was prayed regularly.
When Sr. Alfonsa became ill in 1957, she donated the land to the Archdiocese of Manila. After her death, the Christian community continued the religious activities in the chapel.
The SVD fathers assigned to the Immaculate Conception Parish took turns in administering the chapel to sustain the religious activities of the faithful. After undergoing a major renovation with the help of the lay faithful, the chapel was elevated to a parish church on September 12, 1964 with Fr. Mariano Borlongan as the first parish priest.
